Case 17 History ---- The patient was an 80 year old man with a 4.0 cm right parieto-occipital meningioma. The tumor had eroded dura, and was associated with hemispheric edema. ---- 17A1,2 Sections reveal a meningothelial meningioma with a number of aggressive histologic features. The latter include sheeting (patternless) architecture, focal small cell (high N/C ratio) change, a focally elevated mitotic index of up to 4-5/10HPF and clear cell features. The latter finding is not diffuse and is associated with a perivascular "blocky" collagen deposition only in some regions. Other regions of this neoplasm demonstrate increased vascularity and moderate to marked nuclear pleomorphism.
