Case 24 History ---- The patient was a 15 year old girl who was additionally diagnosed with a bilateral occipital dural-based tumor in 2003. Two years later she presented with multifocal dural-based nodules and a second resection was performed. ---- 24A1-4 Sections from both the 2003 and 2005 resection specimens reveal similar histologic features. The tumor appears dural based with hypercellular sheets of round epithelioid cells with clear cytoplasm and monomorphic oval nuclei with a bland chromatin pattern. The tumor is associated with a dense fibrous/desmoplastic stroma in the background, including occasional large brightly eosinophilic bundles of collagen, consistent with amianthoid fibers. Although in most areas, the mitotic index is low, it is focally increased up to 4/10HPF. Multiple zones of geographic necrosis are also evident. In the vast majority of the tumor specimen, classic meningothelial features are not evident, although there is a vague sense of whorling in some areas.
