Case 17 History ---- The patient was an 84 year old female who died from complications following a colonic perforation and colostomy. Her past medical history was significant for type 2 diabetes, myocardial infarction x2 with pacemaker placement, hypertension and myasthenia gravis Prednisone treated for a year. By history, she was also status-post right hemisphere infarct with no residual motor deficits. Worsening congestive heart failure resulted in right atrial and ventricular thrombus and consequently she was started on Heparin. Guaiac positive stools prompted colonoscopy, after which she developed abdominal pain and free abdominal air. A colostomy was created and she remained on the ventilator post-operatively because of worsening cardiac failure, fluid overload, and acidemia. Finally, she was extubated after family change in her code status had been changed to 'DNR' and expired.
