Case 32 History ---- The patient was a 65 year old male with a history of coronary artery disease, myocardial infarction, hypertension, COPD, chronic renal insufficiency, and BPH who had a six month history of diplopia, dizziness, and clumsiness. A head MRI showed a left petroclival meningioma which was resected. During surgery, a soft vascular mass with the gross characteristics of a meningioma was found in the left posterior fossa with extension into the left middle fossa. His post-operative course was complicated by a waxing and waning mental status, very little left ocular movements, a right INO, absent left corneal reflex, a dense right hemiparesis, and right focal motor seizures. His left extra-ocular movements, left cornea, and right hemiparesis showed some improvement with time. He remained intubated for the remainder of this post-operative course. On post-op day number 11, he had IVC filter placement because deep vein thrombi were found in the left popliteal, left posterior tibial, and bilateral peroneal veins on a venous Doppler study. On post-op day number 15, he became hypotensive and had a cardiopulmonary arrest. ---- At autopsy the weight of the unfixed brain is 1,330g.
