Case 70 History ---- The patient was a 73 year white male with a past medical history significant for type 2 diabetes mellitus, hypertension, coronary artery bypass grafts (1981), and a subcortical cerebrovascular accident in 1987. His present illness began in 1991 when he presented with right-sided paresthesias and weakness, and expressive aphasia. A CT scan of the head showed no acute infarction. On 6/28 carotid angiography was performed which disclosed a right carotid with 99% occlusion and a left carotid 90-95% occlusion. A left internal carotid endarterectomy was performed without complications. Postoperatively, however, his mental status never improved, and he persisted comatose. His mental status was without change, so it was decided to give only supportive care and shortly thereafter he developed cardiorespiratory arrest and died. ---- At autopsy the patient had significant pulmonary emboli. The brain weighed 1180g. ----
70A1-3 Coronal sections demonstrated an intraventricular meningioma.
