Case 21 History ---- The patient was a 20 year old man with a biopsy proven chordoma of the clivus. On MRI and CT scan, the mass extended from the level of the pituitary gland down to the odontoid process and involved full width and length of clivus of the sphenoid bone. Operative procedure: Transoral resection of chordoma with free flap reconstruction and tracheostomy. ---- 21A1-3 Sections show a chordoma with focal evidence of dedifferentiation. The tumor is composed predominantly of polygonal physaliphorous cells (arrow 21A1, 21A2), lying isolated or arranged in single-cell cords and small nests within a basophilic mucinous background.
