A CD68 immunostain highlights the marked macrophage infiltrate in the lateral corticospinal tracts. (CD68 IHC) ---- Not shown: Sections of the frontal, temporal and occipital cerebral cortices show a relatively well maintained complement of neurons with prominent hyaline arteriolosclerosis. Comparison of the precentral and postcentral gyri fail to show loss of neurons or cortical astrocytosis. There is mild neuron loss in the motor nuclei for cranial nerves IX, X, and XII, with occasional axonal spheroids and mild gemistocytic astrocytosis. ---- Neuro Final Diagnosis: Amyotrophic lateral sclerosis
