The hypoglossal nucleus of the medulla demonstrates mild neuron loss and astrocytosis by H&E stain (not shown) which is confirmed by TDP43 immunoreactivity. The medullary pyramidal tracts show axon loss, microvacuolation, and gliosis. Injury to the descending pyramidal tracts within the medulla; the latter findings are also confirmed with Bielschowsky silver and LFB-PAS staining. ---- Neuro Final Diagnosis: Amyotrophic lateral sclerosis, with severe involvement of upper motor neurons
