4B1-3 The precentral gyrus in this case is shown at progressively increasing magnification validating the presence of neuron loss, particular its large neurons, with pallor and astrocytosis. Both the right and left precentral gyri show striking transcortical loss of neurons with marked astrocytic gliosis, reactive small vessel proliferation and rod-shaped microglial proliferation. A rare focus of neuronophagia is seen. In the subjacent convolutional white matter there is marked pallor and gliosis. The immediately adjacent gyri show similar but less severe changes. (H&E)
