1A0 Case 1 History
Case 1 History ---- This 95yo female died of initiation with diagnoses of dementia, congestive heart failure with permanent pacemaker implant, kidney failure and rhabdomyolysis. She had the gradual onset of dementia characterized largely by memory problems with a duration of 11 years in which hallucinations and fluctuations were noted. The clinical dementia rating (CDR) at time of death was 2 with the most likely clinical etiology of her dementia thought to be Alzheimer disease with a possible contribution by dementia with Lewy bodies. ---- At autopsy the unfixed brain weighed 1130g. There was mild to moderate generalized atrophy and moderate dilatation and blunting of the lateral ventricle. The patient had beta-amyloid deposition, TDP-43 proteinopathy, tauopathy and synucleinopathy in addition to small vessel disease. It was thought that ADNC, Lewy body disease, and hippocampal sclerosis were most likely to explain her cognitive deficits.
