Case 3 History ---- The patient was a 77yo male with a 9 year history of dementia thought most consistent with dementia with Lewy bodies and widespread metastasis from a primary colon adenocarcinoma. The clinical cause of death was respiratory failure secondary to adenocarcinoma metastases. ---- At autopsy the unfixed brain weighed 1270g. He had the pathologic findings of diffuse Lewy body disease (DLBD, correlate of Dementia with Lewy bodies - limbic stage), Hippocampal Sclerosis, and Primary Age-related Tauopathy (PART). There was evidence of four molecular pathologies: synucleinopathy, very modest beta-amyloidosis, tauopathy, and very modest TDP-43 proteinopathy in addition to mild small vessel disease. Lewy body disease and PART together with hippocampal sclerosis most likely explain the major cognitive and motor deficits.
