Case 8 History ---- This 96yo female presented initially to the ADRC at the age of 80 with a CDR score of 0.5 and had a very slowly progressive course over 16 years. At death she was severely demented (CDR 3). Although the long duration is atypical for Alzheimer disease, the other features of her dementia were consistent with an Alzheimer disease clinical phenotype. The cause of death was inanition. ---- At autopsy, the fresh, unfixed brain weighed 1125g. External examination revealed mild to moderate atrophy of the frontal, temporal, and parietal lobes. Coronal slicing of the formalin-fixed left hemibrain revealed moderate to severe dilatation of the lateral ventricle. The hippocampus was severely atrophic. According to the recent NIA-AA criteria, this case was classified as A3B3C3 (where A = beta-amyloid stage [range: 0-3]; B = modified Braak neurofibrillary tangle stage [range: 0-3]; and C = CERAD neuritic plaque stage [range: 0-3]), indicating a "high" degree of AD neuropathologic change.
