In this case the occipital lobes are comparably atrophic. ---- Not shown: There are normal nuclear and fiber tract configurations in the midbrain, pons, and medulla, except for the substantia nigra which shows pigmentation only half way to the lateral margin of the midbrain. ---- The constellation of findings in this case (severe neuronal loss throughout the cerebral cortex, neurofibrillary tangles in the para-hippocampal gyrus as well as the neocortex, extensive involvement by diffuse and neuritic plaques, and verified beta-amyloid and phosphorylated tau in these same structures) are diagnostic of Alzheimer disease in this patient with dementia. The number of senile plaques and neurofibrillary tangles meet the Khachaturian, CERAD, NIA/Reagan and NIA-AA criteria for the diagnosis of Alzheimer disease. There were no other findings of any other neurodegenerative disease.
