Case 4 History ---- This 14 year old patient initially presented in March of 2015 with extreme agitation, anxiety, and echolalia. Workup revealed anti-NMDA antibodies in the CSF, T2/FLAIR hyperintensiy of the left hippocampus greater than the right, diffusion restriction of the splenium of the corpus callosum, and a right ovarian mature teratoma, consistent with Anti-NMDAR encephalitis. She received multiple rounds of plasma exchange, IVIG, methylprednisolone, and rituximab, in addition to tumor resection. Since discharge in May of 2015, she received a short course of outpatient rituximab infusions. According to the patient and her father, they have not noticed any alteration in her mental status, i.e. no episodes of confusion, acting differently, or emotional lability. She is doing well (all A’s) in her sophomore year in high school. There has been no reported reoccurrence.
