2E0 Case 2 Denouement
Neuro Final Diagnosis: - Argyrophilic grain disease, mild ---- Neuro Diagnosis Comment: The main neuropathological findings of this case are relatively sparse tau-immunoreactive pre-tangles and tau positive inclusions in both neurons and glial cells (astrocytes and coiled bodies in oligodendrocytes, also called argyrophilic grains when impregnated with silver salts) in the amygdala and entorhinal cortex. The absence of beta-amyloid and neuritic plaques excludes the diagnosis of Alzheimer's disease by Khachaturian, CERAD, and NIA-Reagan Institute criteria. In conclusion, the modest cognitive deficits experienced during life may possibly be explained by argyrophilic grain disease, but the pathologic findings in this case are quite mild even for that diagnosis.
