There is modest neuronal loss and gliosis involving sections of neocortex. No diffuse beta-amyloid plaques and no neuritic plaques are seen; a few tau-immunoreactive neurons, indicating a pre-tangle stage, are noted. No alpha-synuclein positive inclusions (Lewy bodies) and no TDP-43-immunoreactive neuronal cytoplasmic inclusions are present. ---- 4A1-3 Scattered neuronal tau-positive pre-tangles and tangles are seen in the adjacent parahippocampal gyrus where they are accompanied by pTau grains. ---- 4A1 Whole mount of the hippocampus showing patchy PHF immunoreactivity.
