Case 5 History
This female patient was first seen at the age of 67 in August 1986. There was no family history of dementia. Her history was significant for breast cancer, SP mastectomy in 1986, osteoarthritis of the right shoulder and cataracts. Her husband reported that his wife had concerns about her memory (for names of people or places), but he did not feel these memory problems were consistent or interfered with daily life. She was rated CDR 0, no dementia. The patient returned annually for 25 years. Medical history over this time included: 1987 left cataract removal with infection resulting in blindness of the left eye, hysterectomy 1995, hernia repair 1996, Rheumatoid arthritis with prednisone treatment daily 1998, GERD 2000, spinal stenosis surgery 2002, HTN 2002, and increasing physical frailty 2011- vision changes, hearing loss and decreased mobility. For assessments (years #2-22) the patient was rated as CDR 0, no dementia. She completed a T23 assessment in April 2011 at the age of 92 and was rated CDR 0.5, uncertain dementia; however, the next year she was again at CDR 0, no dementia. At her last assessment in 2013 her collateral source described gradual changes in memory over the last year and she was rated CDR 0.5. In 2014 she presented at the ER and was diagnosed with ischemic colitis and renal failure. She refused surgery for the incarcerated hernia, her family supported this decision, and she was sent home on hospice. She died at the age of 95.
