Case 4 History ---- The patient was a 78yo male who initially presented in 1995 at 72yo to the Movement Disorders Clinic (MDC) for evaluation of bradykinesia. At that time he noted diminishment of facial expression and slow movements. He began experiencing stiffness and slowness when getting out of a car. He was at that time diagnosed as having idiopathic Parkinson's disease. He was felt to be somewhat atypical given relative symmetric onset so other etiologic considerations were introduced, such as PSP or striatonigral degeneration. He had significant worsening of his gait and mobility but also had significant worsening eye movements that were felt to be consistent with PSP and/or a multisystem atrophy. He received minimum benefits from Sinemet. He also began demonstrating hemidystonia involving the left side which suggested corticobasal ganglionic degeneration (CBD).
