Case 2 History - (AANP Diagnostic Slide Session 2012, Case 10) ---- The patient is a 38 year-old Caucasian female with a clinical history of chronic headaches and hemochromatosis. The patient presented in 2007 with a grand mal seizure. An MRI revealed a T2 intense and T1 hypointense lesion with irregular peripheral enhancement measuring 2.7 X 2.0 X 2.2 cm centered in the right parietal lobe. A biopsy and subsequent resection was performed and interpreted as showing inflammatory changes. Despite surgical intervention, the patient’s seizures persisted. The patient was followed with serial MRI, noting no change until July of 2011 at which time the lesion was found to show increases in heterogeneous enhancement and extension to the ependyma of the lateral ventricle. She presented for additional evaluation and management which included biopsy. She received a diagnosis of Crystal-Storing Histiocytosis associated with kappa light chain restricted plasma cells.
