Case 2 History ---- This female patient was living in a retirement community and was completely independent until her mid 80s. The collateral source described a slight inconstant problem with memory that started at that time and the change became more noticeable when she was 86 and was hospitalized in a psychiatric unit for visual and auditory hallucinations. Her dementia rating at that time was CDR1 and it was thought that she had dementia with Lewy bodies. At 89 years she was at a CDR of 1 but her memory issues had become more dominant and it was thought she had AD. At the age of 95 she was rated CDR2, still thought to have AD. She expired within 3 days of hospice initiation when she was 95 years old. Her clinical conditions were congestive heart failure, inanition, kidney failure and rhabdomyolysis. In summary, this patient had a gradual onset of dementia, with a duration of 11 years, and experienced hallucinations and fluctuations. The CDR at the time of her death was 2 and her clinicians felt the most likely etiology of her dementia was Alzheimer’s disease with possible Lewy bodies.
