5E Case 5 Denouement
Neuro Final Diagnosis: Alzheimer's Disease, mild to moderate severity; Arteriolosclerosis, mild with patchy white matter pallor; Hippocampal sclerosis, right greater than left with TDP immunoreactive inclusions. ---- Comment: Alzheimer changes involve the same areas which are depopulated of viable neurons and contain scattered ghost tangles. Is this hippocampal sclerosis or Alzheimer-induced neuronal loss, both, or ? Multiple sections of frontal, parietal, temporal and occipital neocortices show a relatively well maintained complement of neurons generally without marked astrocytic and microglial proliferation or parenchymal perivascular inflammatory cells. Pick bodies, cortical Lewy bodies, neoplasia, malformation and ballooned cells are not demonstrated in H&E, beta-amyloid, and phospho-tau stained material of the neocortex.
