2A0 Case 2 History
Case 2 History ---- The patient was a 70yo female with Huntington disease, genetically diagnosed in 1999. She had a slow decline over ten years with movement problems in the last five years. Her symptoms included cognitive impairment, chorea, unsteady gait, limited speech and impaired swallowing. A clinic note from 05/2013, states advanced Huntington disease with increased problems with balance and hand coordination. She was mostly chair-bound and unable to walk without assistance. Her mental capacity was limited with difficulty in following instructions. At that point, exam was notable for strong frontal release signs, mild chorea, minimal verbal output and profound gait instability. In 08/2013, she was sleeping excessively (12-16 hours/day) and also had bowel incontinence. More recently she was admitted at an outside hospital for a fall/ GI bleed in early October 2013 and discharged a few days later. She died a few days later. Family history is notable for Huntington's disease in her mother and sister.
