Case 10 History ---- This 82yo female first noticed left upper extremity and bilateral resting leg tremors at age 73. One year later, handwriting became smaller and less legible, her voice became softer and she became much slower. At age 74, she started to freeze in doorways. She started to fall about one year after symptom onset. Within 2 years after onset, all movements were substantially slowed and gait freezing became quite prominent. Levodopa provided good benefit, except for freezing, but she had early wearing off after developing of leg dyskinesias. She had mild orthostasis but no hallucinations, REM behavior disorder or excessive daytime sleepiness. She had constipation and stress urinary incontinence without depressive symptoms. Exam at age 75 revealed stage 4 parkinsonism, mostly of the lower body. Her disease continued to progress with increasingly difficulty in mobility and very poor speech. Subsequently she died under hospice care from poor oral intake.
