Case 11 History
This 73 year old right handed male first came to the Movement Disorders Center in 1996. At age 55, he first noted stiffness across his shoulders after a skiing accident. At age 56, he developed a resting pill rolling tremor in the left hand. He started to have flexed posturing, particularly when walking. At age 58, he had more staring expression with increased drooling. At that time, it was harder to arise from a chair and speech became less clear and softer. Levodopa provided good benefit. However, motor manifestations progressed over the years. He developed dementia, hallucinations, orthostasis with syncope, excessive daytime sleepiness related to dopa, and some dyskinesias. He had required nursing home placement. Last MMSE was 14/30. Clinical diagnosis: Idiopathic PD with dementia likely due to cortical Lewy body disease. The clinical summary was prepared by:
