Coronal slicing revealed mild dilatation of the lateral ventricles with rounding of the angles; although there was increased space in the temporal horns of the lateral ventricles, the hippocampi appeared of normal size. ---- Not shown: There was widespread brainstem, limbic (>neocortical) alpha-synucleinopathy in the form of cortical Lewy Bodies. Foci of frequent beta-amyloid plaques, no neuritic plaques and very sparse neurofibrillary tangles were present. The findings in this case indicate the presence of Alzheimer's disease (AD) by Khachaturian criteria, but the absence of neuritic plaques excludes AD by CERAD, NIA-Reagan Institute and NIA-AA criteria for the neuropathologic diagnosis of AD.
