Case 4 History ---- The patient was a 71yo male first seen for memory and concentration problems beginning 5 years prior to his death. For the next 2-3 years, he had increasing trouble with losing his place in a conversation, learning how to do new tasks like using a phone card, and attending to personal hygiene. He stopped driving. About 2-3 years after the onset of his cognitive problems, all activities became slower including walking. His gait became shuffling. Over the next 2 years, his handwriting became slower and smaller – as well as less legible. Within another year, he developed drooling and difficulty swallowing. He did not have tremor. In the last year, he had decreasing oral intake and was less interactive. He died from inanition and dehydration while comfort measures with morphine were maintained with hospice care.
