Case 5 History ---- This 66yo female first had the gradual onset of shuffling gait and small handwriting at age 52 years. Her posture became flexed and within one year, she had cognitive problems with “slower processing of verbal commands and reduced awareness of her surroundings. In the second year of her illness, she developed resting tremor of the left hand and then the left leg. Soon thereafter, she had softer speech. She fell and also developed freezing of gait. She had hallucinations by that second year which were associated with levodopa. Over the years, she had increasing trouble with gait instability, falling, freezing, progressive dementia, hallucinations, excessive drooling (responded to btx injections) and orthostasis. Last MMSE in August 2010 was 20/30. She required complete assistance for all ADLs due to motor and cognitive impairments. The clinical cause of death was carcinoma of the lung.
