Case 8 History ---- At age 76 he developed balance problems and later developed right hand resting tremor. At age 77, he had right arm flexed posturing and started to fall on the tennis court. At age 78, he developed visual hallucinations – prior to PD meds. At age 78 he also developed softer speech, impaired handwriting, and flexion of the neck. At age 79, he was slower arising from a chair and walking became slower. At age 80, he developed propulsion and started having freezing. Concentration became impaired and he had some orthostasis. When first seen in a parkinsonism workup at age 80. he had modest benefit from levodopa, orthostasis, persisting visual hallucinations, severe dementia, modest excessive daytime sleepiness, rare REM behavior disorder symptoms, but no depression. About 2 months prior to his death, he developed pneumonia was admitted to hospital, then to rehab and home on hospice care for 36 days. He stopped eating and drinking in the last 5 days and died.
