Coronal slicing revealed mild dilatation of the left lateral ventricle and its inferior horn, indicating mild loss of brain parenchyma. The hippocampus appeared macroscopically of normal size and proportion. ---- Final Neuropathologic Diagnoses: Diffuse Lewy body disease (Parkinson disease with dementia; brainstem, limbic and neocortical); Alzheimer's disease neuropathologic change (Low: A3, B1, C1). ---- Comment: This case displays alpha-synuclein-positive inclusions in the form of Lewy bodies and Lewy neurites in brainstem, limbic and neocortical areas. When AD and DLBD are seen in one patient, as here, there is a ‘high probability’ that the Lewy body pathology contributed to the dementia (McKeith et al, PMID: 16237129).
