Neuro Final Diagnoses: Idiopathic Lewy body Parkinson's disease (S/P right pallidectomy and bilateral subthalamic stimulator placement); Diffuse subarachnoid hemorrhage (S/P motor vehicle accident); Cerebral edema ---- Not shown: Although rare apparent cortical Lewy bodies were noted in the H&E stained cingulate gyrus, ubiquitin immunohistochemical studies fail to show significant numbers of cortical Lewy bodies. The subthalamic nuclei show reactive changes surrounding the stimulator insertion sites, characterized by focal tissue loss, accompanied by reactive gliosis and hemosiderin-laden macrophages with small numbers of axonal spheroids. The substantia nigra and locus ceruleus showed neuron loss, astrocytosis and Lewy bodies.
