Neuro Final Diagnosis: Idiopathic Parkinson's disease, s/p electronic stimulator implantation, bilateral subthalamic nuclei. ---- Not shown: The substantia nigra and locus ceruleus are markedly depigmented. Multiple scattered diffuse and neuritic plaques are identified by H&E and Bielschowsky stained sections. However, the number of the plaques do not meet Khachaturian or CERAD criteria for Alzheimer's disease. No cortical Lewy bodies are noted. The implantation sites of the electronic stimulators and their tracts are seen as cystic spaces lined by gliotic tissue. Several residual pigmented neurons in substantia and locus ceruleus contain Lewy bodies. Autopsy examination confirms the clinical diagnosis of idiopathic Parkinson's disease and reveals that the electronic stimulators were appropriately implanted in the subthalamic nuclei, consistent with the clinical improvement of the patient's dyskinesias.
