Neuro Final Diagnoses: Diffuse Lewy body disease; Alzheimer's disease Neuropathologic Change ADNC); Organizing ablation/stimulator sites and tracts, remote:(Right globus pallidus; right and left subthalamic nuclei ---- Comment: The major findings in this case is the presence of Lewy bodies in the substantia nigra, locus ceruleus and cortical Lewy bodies consistent with a diagnosis of diffuse Lewy body disease. Numerous neurofibrillary tangles, and diffuse and neuritic plaques are demonstrated in H&E and Bielschowsky stained material, primarily in the temporal lobe/hippocampus sections. The number of senile plaques does meet the CERAD criteria for the diagnosis of Alzheimer’s disease.
