Case 12 History ---- The patient was a 73yo male with past medical history of congestive heart failure, hypercholesterolemia, and dementia. His neurological history is notable for TIA's in his late 50s (1985). His cognitive function was normal until 1998 when his wife and daughter first noted some mild word finding difficulty and decreased verbal output. In early 1999 the patient was started on donepezil and was still able to function independently. In May of 1999 during an episode of stomach pain, he developed paranoia regarding strangers being in the house and started speaking of deceased family members as if they were still alive. He was first seen in the memory and aging center in July of 1999 at which time he was no longer able to wash, groom, or dress without assistance. Psychometric testing was consistent with a mild to moderate dementia with profound aphasia. In September of 1999 the patient was placed in a nursing home. At that time he required Risperidone for agitation and had become a fall risk. He then developed pneumonia requiring hospitalization. His mental status never improved and he remained in a nursing home until he expired from respiratory failure due to pneumonia and congestive heart failure while receiving hospice care.
