Case 4 History ---- This 77yo female was seen once at the MDC at age 67years. In January 2000 her brother, who served as Collateral Source (CS), noted she was complaining of having trouble balancing her checkbook. When he went to her apartment, she was confused, was disoriented to time, and was having trouble writing checks. Other difficulties were noticed, she was not driving well, and it was thought that she was having visual hallucinations. Her MMSE score was 10. She also had a brain biopsy from the right frontal region in March 2000. There was no evidence of infection or vasculitis, but both diffuse and neuritic plaques were seen. She entered a nursing home and her condition stabilized. At the time of the MDC visit, she was having short-term memory problems. Her MMSE score was 13 and Short Blessed score was abnormal at 21. Other psychometric testing showed poor performance in short-term memory, executive function, and fluency tests. Her neurological exam revealed masked facies and other mild features of parkinsonism. There was a mild resting tremor, mild cogwheel rigidity, and small fast finger movements. The clinician thought she had a CDR of 2. She died in July 2010 of end stage dementia and inanition at age 77 years. She was mute, non-ambulatory, and required total nursing care (CDR 3). The cause of death was inanition.
