The cerebral cortical grey matter contains large numbers of neuritic and numerous non-neuritic plaques as well as scattered neurofibrillary tangles by Bielschowsky silver stains. Large numbers of intraneuronal eosinophilic structures, consistent with cortical Lewy bodies are seen in the temporal, frontal and especially cingulate cortices. The substantia nigra shows neuron loss and gliosis as well as several Lewy bodies. No Lewy bodies or substantive neuron loss are seen in the loci cerulei. ---- Final Neuropathologic Diagnoses: Diffuse Lewy Body Disease; Alzheimer's disease ---- Comment: Neuropathologic examination of the central nervous system shows idiopathic Parkinson's disease with Lewy bodies in the substantia nigra as well as large numbers of cortical Lewy bodies. In addition, large numbers of neuritic plaques are seen in the cortical grey matter and, together with patient's clinical history of severe dementia, indicate a diagnosis of Alzheimer's disease.
