Case 1 History ---- This 71yo male was first seen in the Movement Disorders Center (MDC) in March 2006, 3 years prior to his death. His wife reported that his symptoms began with memory and concentration problems 5 years earlier. About 2-3 years after the onset of his cognitive problems, all activities became slower, including slower walking. His gait became shuffling. Over the next 2 years, his handwriting became slower and smaller – as well as less legible. Within another year, he developed drooling and difficulty swallowing. Soon thereafter (now about 4-5 years after onset) he developed visual hallucinations and paranoid delusions. Follow up demonstrated substantial progression of the dementia with a MMSE of 9 of 30, 2 years prior to death. Motor and cognitive problems progressed with asymmetry of the motor manifestations and modest response to levodopa. Hallucinations continued. In the last year, he had decreasing oral intake and was less interactive. He died from inanition and dehydration.
