1C There are scattered beta-amyloid containing largely diffuse plaques. (BAP IHC) ---- In the staging scheme proposed by Braak et al. the presence of Lewy bodies in brainstem, limbic and neocortical areas in this brain is consistent with Parkinson's disease stage 6 (range: 0, 1-6, Braak et al. PMID: 12498954). Neocortical areas show frequent beta-amyloid plaques, only very sparse neuritic plaques and only a few neurofibrillary tangles, concentrated in the medial temporal lobe, not consistent with a major contribution to dementia by CERAD and NIA-Reagan criteria. By Mckeith et al. criteria (PMID: 16237129) which combines both PD and AD pathology, there is a 'high probability' that the dementia is caused by Lewy body disease. ---- Neuro Final Diagnosis: Idiopathic Parkinson’s disease with Lewy body dementia; Alzheimer's disease-type changes (mild)
