2A0 Case 2 History
Case 2 History ---- The patient was a 76yo male who began to have left hand resting tremor, followed by shuffling gait, drooling, diminished arm swing and freezing of gait over 7 years. A year prior to his death, he had stage 4 PD with relative symmetric rigidity and bradykinesias, a history of asymmetric tremor, marked hypophonia, masked facies, moderate to severe freezing, swallowing problems and orthostasis. He also had clear evidence of dementia with impaired ADL’s. MMSE at that time was 22. He continued to have severe orthostasis and hallucinations/delusions. He had recurrent bouts of orthostatic hypotension and following one episode he suffered cardiac arrest and resuscitation was not performed, as per his advance directive.
