Case 5 History ---- This 74yo male was seen at the Memory Diagnostic Center (MDC) in March 2002 at age 68 years. The Collateral Source (CS) stated that the patient had a memory and thinking problem processing information. He had difficulty with recent memory in that he was repetitious. His language was intact. There was no difficulty with orientation and he was still driving without problem. His MMSE score was 23 out of 30. The clinician rated him CDR 0.5 with a diagnosis of dementia of the Alzheimer type. At age 70, his wife stated he had severe short term memory loss and would forget conversations consistently. His MMSE score was 18 and SBT score was 19. All psychometric test scores had declined and his CDR rating was 1, DAT. He had delusions. There was no indication of stroke or Parkinson’s disease. His CDR was 3 with a diagnosis of DAT. He fell down the steps to his basement and was hospitalized with a diagnosis of subdural hematoma which his family chose to treat with comfort measures only. Before his fall, he was only speaking gibberish, He died in October 2007 at age 74.
