8A0 Case 8 History
Case 8 History ---- The patient was an 89yo female. At the T1 assessment, the CS, a daughter, reported a 10 year history of mild forgetfulness that began slowly and had progressed slowly. The patient forgot recent conversations, repeated questions/statements, had traffic accidents and misplaced items. The examiner rated the patient CDR 0.5, dementia of the Alzheimer’s type. She had relinquished driving, limited cooking to heating meals in the microwave, and required prompting for bathing and grooming. She had moved to a nursing home in late 1998 due to increasing difficulty with incontinence. She died in September of 2007 at the age of 89 due to inanition. The patient had been moved to a new nursing home in August 2007. She had stopped eating or drinking for about a week prior to her death. In discussion of PD symptoms, the Collateral Sources described hand tremor when writing or moving her hands and unsteady gait changes, both noted in 1998. There was no significant improvement of PD symptoms with the use of Sinemet. The terminal CDR was 3,
