Few neurons remain in the ION. (H&E) ---- Not shown: GCIs are present throughout the midbrain, including the substantia nigra and red nucleus and are very numerous in the crus cerebri. No Lewy bodies are seen. There is severe neuronal loss in the Purkinje cell layer and more modest loss in the dentate nucleus. The deep white matter shows scattered GCIs. ---- Neuropathologic Findings: Multiple system atrophy (by Gilman et al. PMID: 10223419 criteria this is parkinsonian-type; MSA-P) ---- Comment: Lewy bodies and Lewy neurites are not present anywhere in the neuraxis so Parkinson disease is excluded as a possible diagnosis. There is no evidence of Alzheimer disease (AD).
