Case 1 History ---- This Movement Disorders Center (MDC) male patient developed problems with balance, micrographia, soft voice, masked facies, tremor, and erectile dysfunction at age 62. In the first year he also developed nightmares with screaming and kicking in his sleep. Over the next year, he had progressive bradykinesia and rigidity (R>L). Levodopa at substantial doses provided minimal benefit. He developed orthostasis, more urinary problems and had difficulty swallowing with rapid progression over his last 6 months. He had no evidence of cognitive dysfunction (MMSE was 30/30). He died at age 65.
