Neuro Final Diagnosis: ---- Multiple system atrophy (Parkinson subtype, MSA-P) ---- Not Shown: The cerebral hemispheres showed numerous alpha synuclein-positive oligodendroglial inclusions (GCI) in the white matter and deep cortex and absence of beta-amyloid plaques (BAP) and neurofibrillary tangles (NFT). The amygdala and entorhinal cortex also have scattered GCI. The thalamus shows little GCI other than the included white matter. There is moderate loss of neurons, gliosis and extracellular pigment in the substantia nigra with numerous GCI. There is evidence of neuronal loss in the medulla, including the inferior olivary nucleus. Many GCIs are seen in the medulla, particularly the pyramids. There is cerebellar white matter pallor, axon loss, gliosis, and multiple GCIs. Lewy bodies were not present anywhere in the neuraxis excluding a diagnosis of Parkinson's disease.
