The midbrain is remarkable for the loss of large numbers of neurons from the substantia nigra zona compacta with pigment, both free and in macrophages, astrocytosis and absent Lewy bodies. The decussation of the superior cerebellar peduncle and neuronal complement in the red nucleus are well preserved. The oculomotor nuclei are similarly well preserved.
