Near complete loss of Purkinje cells is accompanied by impressive Bergmann astrocytosis as well as astrocytosis and microglial/macrophage proliferation. These changes are likely subacute in nature, although some may reflect evolving subacute-chronic hypoxia/ischemia. ---- Comment: Microscopic sections of all of the sampled areas of brain show marked superimposition of hypoxic/ischemic changes consisting of patchy neuron loss with residual neurons undergoing eosinophilic neuronal necrosis, astrocytosis, axonal spheroids, microglial proliferation, rod cell formation and neuronophagia. These changes were superimposed on her underlying neuropathology has made it very difficult but not impossible to appreciate her baseline PANK2 related pathology which is overwhelmed by hypoxic/ischemic damage.
