Case 10 History ---- The patient was a 75 year old male who at the first assessment had a one-year history of gradual deterioration in terms of memory for important information. He would increasingly forget daily event, people's faces and appointments. In addition to the above changes, possible depressive symptoms existed including thoughts about dying and a 30-lb. weight loss. Also, this gentleman was physically frail in terms of significant emphysema and COPD. At his first visit in May 1991 he was CDR 1 by the examining physician who thought the patient had AD. The patient was seen for the last two years of his life and showed typical, gradual progression to a CDR 3. He had become dependent in all areas of care, became mute, and it was questionable whether or not he recognized his spouse. His medications prior to death included Sinemet, for possible Parkinson disease. The participant died of respiratory distress at the age of 75. He died with a CDR 3 stage thought to be DAT.
