Similarly the red nucleus has lost neurons, become gliotic and contains occasional tangles. (H&E) ---- Not shown: Sections of cerebral cortex show a diffuse, mild increase in astrocytes and microglia. Silver stains demonstrate numerous senile plaques and occasional neurofibrillary tangles. Examination of the brainstem, cerebellum, and basal ganglia demonstrates widespread presence of argyrophilic tangles. Tangles are numerous in the globus pallidus, subthalamic nuclei, and ventral pontine nuclei. They are also easily found in the substantia nigra, red nuclei, peri-aqueductal grey, locus ceruleus, dentate nucleus, and thalamus. Scattered tangles are also seen in the caudate and putamen. Neuron loss and gliosis is present in the globus pallidus, subthalamic nuclei and dentate nuclei. No Lewy bodies are seen. ---- FINAL DIAGNOSES: 1) Alzheimer disease neuropathologic change. (slides not available for current evaluation but appear not to be compelling for this diagnosis); 2) PSP.
