(Synaptophysin IHC) ---- Neuro Final Diagnosis: Progressive supranuclear palsy (PSP); Alzheimer disease pathological changes (mild) ---- Neuro Diagnosis Comment: The overall neuropathological findings are consistent with progressive supranuclear palsy, as demonstrated by widespread distribution of tau-immunoreactive tangles, tufted astrocytes, and coiled bodies. The involvement of the dentate nucleus, the lack of swollen neurons in the cortex and the presence of tufted astrocytes (specific for PSP) would argue against a diagnosis of corticobasal degeneration. There are also mild Alzheimer disease pathologic changes, but the extent is limited and is unlikely to contribute to the clinical picture in this patient. There is no evidence of Lewy body disease involving the brainstem, allocortex or neocortex.
