The dentate neurons are undergoing grumose degeneration. PHF-1 IHC reveals tangles, pre-tangles, and a mild-to-moderate density of neuropil threads within the dentate nucleus, and occasional tau positive threads and ‘tufted’ astrocytes in the cerebellar white matter. (H&E) ---- Not shown: Sections of the left frontal cortex and occipital cortices show mild arteriolosclerosis, but no convincing evidence of BAP plaques. PHF-1 IHC highlights frequent neurofibrillary tangles and neuropil threads and occasional tufted astrocytes in the inferior olive and reticular formation. No evidence for Lewy body disease, corticobasal degeneration, multiple systems atrophy, Alzheimer disease, or other neurodegenerative diseases is appreciated. ---- Neuro Final Diagnosis: Progressive supranuclear palsy
