Case 3 History ---- The patient was a 78yo male with a past medical history significant for CAD with bypass and PSP diagnosed 6-8 years (2002-2004) before death characterized by difficulties with balance, deterioration in handwriting, and visual problems. Five years prior to his demise he fell and broke a femur which healed well but left him dependent on a walker. Between 2006 and 2007 he developed difficulties with chewing and swallowing, lost the ability to speak, and his dexterity deteriorated. In 2008 he experienced weight loss and aspirated, and an G tube was placed. He subsequently developed urinary incontinence and in 2009, he aspirated a few more times and developed pneumonia. A J tube was placed in March of 2009, and he was sent home with hospice care. In mid-September he developed respiratory difficulty and passed away shortly thereafter.
